The MHN Accountable Care Organization (MHN ACO, LLC), a network made up of 16 Cook County non-profit provider organizations, announced today Yasin Patel will serve as president and CEO of the organization. Patel brings over a decade of experience in the healthcare industry and is an expert in value-based care and clinical quality innovation.

The MHN ACO is comprised of 13 federally qualified health centers and three hospital systems who provide care for more than 180,000 Medicaid and Medicare patients. The network contracts with payor partners to improve health outcomes for Medicaid and Medicare patients in Cook County through a holistic approach that integrates medical, behavioral, and social care.
As the president and CEO, Patel will work closely with long-time partner Medical Home Network, which founded the MHN ACO in 2014 and recently renewed its management services agreement to provide care enablement services for the ACO.

He also managed value-based care contracts with hospitals, health systems, and provider groups nationally as part of the Center for Medicare and Medicaid Innovation’s (CMMI) Bundle Payment for Care Improvement Advance (BPCIA) program.
